The Patient Representative or designated volunteer may visit during an inpatient stay at Fairfield Medical Center, to ensure that your visit is as comfortable as possible.Should you have any questions or need additional information, please call the Patient Representative at extension 8555 on weedkays from 7:00 am to 5:00 pm. Messages left on voice mail are answered as promptly as possible.
Advanced Directives
Fairfield Medical Center provides Advance Directive documents regarding Living Wills and Durable Power of Attorney for Healthcare forms or information upon request. This information can assist you in completely the necessary documentation, to request this information patients can speak with their nurse or call (740) 687-8660.
