Give the gift of health. When you give to Fairfield Medical Center Foundation, you are contributing to the health of your community. With the generosity of our donors, Fairfield Medical Center is better able to provide the necessary facilities, equipment, medical education and care for every patient.
Your gift is 100% tax-deductible and may be directed to one of our many specific causes or funds, or you may choose to give your gift without restriction, allowing Fairfield Medical Center to use the funds as best needed for the benefit of our patients and community health.

Your cash gift is an immediate way to show your support, and you are able to watch your gift being used for the benefit in which you gave. Donations are accepted by cash, check, money order, credit card (Visa, MasterCard, Discover) and can be made in person, by mail or by calling the Foundation at (740) 687-8107.
So often, a person wishes he or she could give to a charitable cause but funds may not be available at that time. Fear of putting themselves in a financial crisis is a real concern. Planned giving gives you the option to fulfill your wishes by leaving a legacy. We have heard from donors that planned giving is about a choice. It is a lifestyle and loyalty for them. It is not about wealth.
There are several types of planned gifts that you can make to Fairfield Medical Center, no matter the size of your estate. A well planned will is the most important document to consider when thinking about estate planning. It coordinates your final wishes. We encourage you to discuss your planned gift with a qualified advisor. Planned gifts include: bequests, life insurance, real estate, IRAs and securities.
Please notify the Fairfield Medical Center Foundation when you have included us in your will. We would very much like the opportunity to thank and recognize you for your generosity. To leave a tax-deductible gift to the Fairfield Medical Center Foundation for the benefit of Fairfield Medical Center and its patients in your will, consider the following:
“I leave and bequeath unto the Fairfield Medical Center Foundation, the sum of $____, or ___percent of my estate (or specific securities or other property). Said Foundation is a nonprofit 501(c)(3) corporation (Federal ID number 20-8952945), organized under the laws of the state of Ohio, with administrative offices at 401 North Ewing Street, Lancaster, Ohio 43130. This gift is irrevocable and unrestricted for general Foundation purposes.”
Once you become a planned giver you join the Legacy Leader’s Society. After making a gift in your will to the Fairfield Medical Center Foundation, you are honored on the Legends & Leader’s Donor Wall located in the South Lobby of Fairfield Medical Center. We will honor the wishes of those who prefer to remain anonymous.

Bequests are the most common and simplest means for planned giving and provide many ways to fulfill legacy wishes.
- Honorary or memorial bequest This is a gift given "in honor of" or "in memory of" someone. We are pleased to honor your request and have many ways to grant appropriate recognition
- Endowed bequest This bequest allows you to restrict the principal of your gift, requiring us to hold the funds permanently and use only the investment income they generate. Creating an endowment in this manner means that your gift can continue giving indefinitely. In order to establish a Named Endowment Fund, a donor must give a minimum of $25,000.
- Restricted bequest This type of gift allows you to specify how the funds are to be used. Perhaps you have a special purpose or project in mind.
- Unrestricted bequest This is a gift for our general purposes, to be used at the discretion of our board. An unrestricted bequest, without conditions attached is the most useful, as it allows us to determine the wisest and most pressing need for the funds at the time of receipt.
Life insurance provides an “amplified” gift that enables the donor, through a relatively small annual cost, to deliver a benefit far in excess of what would otherwise possibly be available for charity. Life insurance plans are a very important part of Fairfield Medical Center Foundation’s planned giving program. We encourage you to consider one of the following:
Transfer of an older policy: If you find that you no longer need an older policy, you can name Fairfield Medical Center Foundation as the beneficiary and irrevocable owner of the policy. You must first transfer the beneficiary’s name to the Foundation before it can be considered a charitable gift. The life insurance gift will be based on the value at the time of receipt. Simply making FMC Foundation the beneficiary of your life insurance policy, keeps all other assets earmarked for family members intact.
Funds to purchase a policy: Donors can contribute future premium payments on a life insurance policy of said donor. Your entire premium payment is considered a gift in the year it is made. Should the donor elect to discontinue future payments of the insurance policy, the Foundation may:
Continue to make the premium payments
Convert the policy to paid up insurance
Surrender the policy for its current cash value
Life Insurance policies are not recorded as gifts until such a time the gift is irrevocable.
The Fairfield Medical Center Foundation may consider gifts of real property (single family residences, farms, vacant land,) by working with the donor and a financial advisor. Upon completing a gift acceptance form, the Foundation may require an initial environmental review be completed. Supporters of the Foundation are encouraged to consider naming Fairfield Medical Center Foundation a beneficiary of your retirement plan or IRAs. Distribution of your gift will avoid both estate and income taxes. The IRA must be directed solely to the Fairfield Medical Center Foundation, avoiding pass through of the estate to avoid taxable gain. The solution is to distribute directly from the IRA to the Fairfield Medical Center Foundation.
If you have enjoyed gains in the stock market and other investments that have increased in value since purchase, you may wish to consider giving it as a charitable contribution this year. When you give to the FMC Foundation by gifting publicly traded securities, real estate, collectibles, etc., you are helping sustain local community healthcare with little or no effect on your current spending income. GRATEFUL PATIENT PROGRAMS
Patients, friends and families can make a contribution to the FMC Foundation in honor of an outstanding caregiver. The grateful patient program is a means to go beyond saying thank you. With your gift in honor of your physician, nurse, volunteer, chaplain, etc., you can elevate them to Legendary Caregivers, helping them to earn their angel wings pin. An array of possibilities exists for naming opportunities at Fairfield Medical Center. Your gift in naming one of the many spaces within the Center provides you with the opportunity to leave a legacy of your family name or someone for whom you deeply care. Lectureships & Lecture Series:
Fairfield Medical Center Foundation encourages naming opportunities for distinguished lectureships and visiting lecture series. Lecture series provide Fairfield Medical Center with individuals who bring expertise and experience to special interests of a particular subject or medicine. Lectureships provide funding for presentations in the Graduate Medical Education department for residential training.
Scholarships enable us to provide employees with assistance in completing their medical education to benefit our patients. Scholarships are presented to employees who are furthering education in their related field, allowing us to continue to provide the excellent care and service to which FMC is committed.

MEMORIAL & CELEBRATORY GIFTS
Families can establish an endowed memorial or celebratory fund in the name of the decedent or honoree with gifts totaling $25,000. Endowed funds increase in market value over time as only a portion of the fund’s income is spent annually. The fund will exist in perpetuity, which serves as a nice memorial to your loved one.
The entrance to the North Lobby of FMC serves as a beautiful location for our brick memorial patio. If you have a loved one or someone you wish to recognize, you may choose to engrave their name(s) onto a choice of bricks.
4” x 8” - $250 (3 lines with a total of 48 letters per brick)
8” x 8” - $500 (3 lines with a total of 96 letters per brick)
Bricks can be purchased at the Fairfield Medical Center Foundation office.

The Fairfield Medical Center Foundation has two signature events per year: the Annual Food & Wine Tasting and the Annual Golf Outing. During 2013 we are also supporting the Heart and Lung Race. If you are an employee of a company that has a program that will “match” your contribution to the FMC Foundation, you can multiply your gift, making it go further.
