Grateful Patient & Family Program

Our patients and families often ask us how they can say “thank you” for the outstanding compassion and care provided at Fairfield Medical Center. Perhaps you’ve already expressed your appreciation with a hug, handshake or note, but feel you want to do something more. If so, I invite you to participate in the Grateful Patient & Family Program.

The Grateful Patient & Family Program is a meaningful way to show your thanks and celebrate the extraordinary care received by you or your loved one while here at Fairfield Medical Center. If you would like your gift to honor a physician, nurse, staff member or volunteer for the care you or a loved one received, you may recognize that person as a “Legendary Caregiver.”

At Fairfield Medical Center, we are committed to providing you and all of our patients with the most exceptional care possible. By partnering with the Foundation, you not only have an opportunity to leave a legacy — you can save a life.

How the Program Works

The caregiver will receive a card with a special note notifying him or her of your gift and gratitude. Your gift, large or small, is powerful. It allows us to continue to invest in the latest healing technology and offer legendary care to every patient without exception. You may designate your donation to a specific area, or make your gift unrestricted, which will be used in the area of greatest need at the Center. Donations made to this program aid in providing our physicians and staff with tools and resources necessary to continue giving the most extraordinary care available.

From new equipment to specialized patient care, your gift will help Fairfield Medical Center fulfill its mission of providing high-quality healthcare to every patient who walks through our doors.

Please take a moment and click here to print and complete this donation card in honor of someone who provided exceptional care to you or your family member. You can complete the card and mail it in, or donations can also be called in over the phone with a credit card. You could be the one who makes all the difference.
