Welcome to Fairfield Medical Center Medical Staff Services’ website. We are your resource for Medical Staff Bylaws, Rules and Regulations and Medical Staff Department and Committee functions. We are also responsible for coordinating the credentialing process for Physicians, Certified Nurse Practitioners and Physician Assistants. You may call us at our main number (740) 689-6454. PRACTITIONER VERIFICATION

CREDENTIALING: Practitioner Eligibility Requirements

Only Practitioners with an active unrestricted license to practice in the State of Ohio who can document their background, experience, training and demonstrated competence, their adherence to the ethics of their profession, their good reputation and their ability to work cooperatively with others with sufficient adequacy to assure the Medical Staff and the Board of Directors that any patient treated by them in the Medical Center will be given a high quality of medical care, shall be qualified for appointment to the Medical Staff. A Practitioner must also provide evidence of satisfying requirements for board eligibility as well as evidence of certification or re-certification as applicable. Further, all Appointees of the Medical Staff must meet the continuing education requirements established for Practitioners as applicable to his or her licensure in order to be eligible for and continuously maintain licensure by the State of Ohio; maintain, at all times, professional liability insurance as required by the Center’s Board of Directors; and comply with state and federal law.

Once we have received your completed application packet, the verification process takes an average of 30 days. It is important to provide complete information in order to expedite the verification process. After the verification process is complete, your application will be channeled through the medical staff approval process.
