Medical Information Services

Fairfield Medical Center maintains medical records and patient information in a private and secure manner in accordance with federal, state and medical record keeping guidelines. Medical records are maintained with the highest degree of accuracy utilizing modern technology and nationally recognized quality processing. The Medical Information Services Department provides information for Trauma and Cancer Registries, and maintains certification status after meeting rigid criteria for record keeping practices. Many of the staff in the Medical Information Department team are credentialed by the American Health Information Management Association while others have extensive electronic medical record keeping experience. Our 24 hour record keeping operation provides timely record keeping for our clinicians and caregivers.

Release of Information

Our Release of Information Section is open Monday – Friday from 8:00 a.m. to 7:00 p.m. Bring a picture identification and appropriate guardianship, power of attorney or guardianship documents when coming to the Center to obtain medical information. Download the “Authorization for Release or Disclosure of Medical Information” form. The cost for copies of medical records are determined based on retrieval fees and the number of pages.

Medical Transcription

The Medical Transcription Section provides transcription services for the medical and clinical staff to document medical reports documenting patient care. The medical transcription team consists of highly experienced and trained medical transcriptionists most having more than 20 years of transcription experience. Our team are specialized in imaging, pathology, surgery, emergency and acute care report.

PBX/Switchboard is responsible for answering and directing calls to the correct person or department. They monitor our camera system and control access to departments after hours. The operators also call our codes overhead and notify the appropriate people to respond to those codes. This department is the communication hub for Fairfield Medical Center.

The Registration/Admitting department is staffed to serve our patients 24 hours a day and 7 days per week. Their responsibilities include documenting accurate demographic and insurance information to ensure appropriate and accurate billing of services. Registration also admits Inpatients to the hospital and pre-registers patients for future services.
