What is Fairfield Community Health Partners ACO?
Fairfield Community Health Partner is Fairfield Medical Center’s Medicare ACO. ACOs are groups of doctors, hospitals, and other healthcare providers who come together voluntarily to give coordinated high-quality care to their Medicare patients. The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time while avoiding unnecessary duplication of services and preventing medical errors. Our doctors and hospitals in the Fairfield Medical Center ACO communicate with you and with each other to make sure that you get the care you need when you are sick and the support your need to stay healthy and well.
