Staying By Your Side After a Hospital Visit

Getting back on your feet after a stay in the hospital or visit to the emergency department can be overwhelming. The Fairfield Healthcare Professionals Continuity of Care Clinic (COC) will help you navigate recovery after your discharge by allowing our care team to remain by your side throughout your health journey, keeping you on track to feeling your best.

Conveniently located near FMC’s main campus, and with telehealth availability, the COC is designed to be there for you as you transition from hospital care back to everyday life.

You may be notified during your hospital visit if you are eligible for an appointment at the COC. Our team may also contact you within a few days of your discharge to see how you’re feeling, answer your questions and schedule an appointment to be seen at the COC soon after your release.

Ensuring Continuity of Care by:

Supporting you with thorough follow-up appointments
Providing regular communication with a healthcare provider
Keeping you out of the emergency department with routine monitoring
Helping you better understand your discharge instructions, medications, treatment regimens, etc.
Coordinating your care plan with primary care providers and/or specialists for continuation of long-term care

Dedicated Continuity of Care Provider

The provider you see at the COC will support you with short-term follow-up care, helping you stay healthy after a trip to the hospital. After completing the follow-up care at the COC, we will ensure you are scheduled with a primary care provider and/or necessary specialists in a timely manner to manage your long-term health and wellness.
