Tyler’s goal as a nurse practitioner at the Continuity of Care Clinic is to streamline the transition from hospitalization to home, providing clarification, education and resources to recently discharged patients. Tyler feels that providers and patients need to work as a team – they need each other to be successful!
When she is not seeing patients, Tyler loves gardening, traveling, weightlifting and spending time with her husband and son.
