Planning Actions to Improve the Community’s Health
The Fairfield County Community Health Assessment provides us with a snapshot of Fairfield County, as well as our state and nation. It identifies key health needs and issues through systematic, comprehensive data collection and analysis.
The purpose of the community health assessment is to learn about the community:
- The health of the population
- Contributing factors to higher health risks or poor health outcomes of identified populations
- Community resources available to improve the health status of our county
This valuable data gives FMC comprehensive insight into the community’s current health status, needs, and issues. Further, it can help with developing strategies that focus on wellness, access to care, and unmet community needs by helping determine how and where resources should be allocated to best meet our community’s needs.
It is our hope that this assessment will foster new collaborative opportunities and initiate quality programs to improve the lives of Fairfield County residents.
