The AC Coaches Club is accepting $10 donations the day of the event to continue to supporting AC athletic programs. Please remain in your vehicle until your appointment.

Complete the 2024-2025 pre-participation physical evaluation form from OHSAA, which is found at www.ohsaa.org. This document must be signed and completed by the parent prior to the scheduled appointment. Athletes 15 and younger will need to be accompanied by an adult during the exam.
