My FMC Portal will help you manage your health and wellness by keeping you connected.

This new portal offers patients a secure environment to perform functions related to their healthcare. This application allows patients and their authorized family members to play a more active role in managing their health and wellness. My FMC Portal also allows FMC staff to appropriately access their records.

How Do I Sign Up?
At your next scheduled visit you can provide your email address to a Fairfield Medical Center team member. They will be able to send you a secure email invitation to sign up for My FMC Portal. Through this email invitation you are able to create an account. This invitation to join expires after 90 days.

What if I don't have a scheduled appointment in the near future?

If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m. The staff members in the Medical Records department will be able to walk you through the process to sign up to the new portal.

Why do I have to go to an FMC location to sign up?
At Fairfield Medical Center your safety is our top priority. Secure email invitations will only by sent in person to verify identification through a photo ID. This is done to protect your patient health information.
