New Mobile Registration and Check-In

Fairfield Medical Center is partnering with Phreesia, a patient intake system, in order to improve the patient experience by offering convenient online registration, check-in and bill pay. Mobile check-in allows you to complete pre-visit registration from your own device (any smartphone, computer or tablet) ahead of time and in the privacy of your home. Patients will be able to make payments through a secure online method, message office staff with questions and receive pre-appointment instructions. In-person registration will still be available upon request.

This change will be implemented in waves at select FMC and Fairfield Healthcare Professionals locations until the process is complete at all locations in 2024. You will receive a registration link to your most recent cell phone number or email address. Talk with your provider’s office for any questions regarding this change.

  • No. There is no app – you simply click the link provided in the text message or email. This allows you to securely answer any registration and medical questions necessary for your appointment.

  • Yes, mobile registration is available in 20 different languages.

  • If you have not completed your registration before the visit, see one of our receptionists, and they will be happy to send you a new link. Don’t worry, the information you entered previously will be saved. If you would prefer to not complete the registration on your device, you will still be able to fill out paper forms upon arrival.
